Log all Kills and Glines.

Hey, i have looked around and cant find a verry simpel script that just Logs Kills and Glines. I want it to log it to a .log file something like this,

#Kill By:Nick User Reason
#Gline By:Nick User Reason

i hope someone know how to do this.
